Resale Certificate Request Form

I hereby request a Resale Certificate from Glen Forest Homeowner's Association for the identified property.

I understand that the $75 Transfer Fee must be received before the Resale Package can be provided,  that it is non-refundable, and that I must submit it via USPS.

Please note the property address and "Resale Cert" in the memo portion of the payment.
